Full-Time Mentor - Adult Day Services - M-F Days

This role supports the delivery of person-Centered adult day services by directly assisting individuals with disabilities in planning activities, achieving life skills goals, and fostering independence. Responsibilities include providing emotional support, supervision, personal care, and maintaining required documentation related to individual service plans.

A high school diploma is required, though 2-4 years of related experience is preferred, or an equivalent combination of education, experience, and training. Candidates must possess strong interpersonal skills, including empathy and active listening, and be able to perform physical tasks like lifting up to 50 pounds and assisting with personal care.
