Core responsibilities

This role involves implementing and maintaining a recognized diabetes self-Management education and support (Dsmes) program, working closely with diabetic patients to assess physical health, psychosocial issues, and identify educational needs based on national standards. The educator will develop and implement diabetes care plans collaboratively with providers and educate patients/Caregivers on self-Care practices like disease process, nutrition, and medication management.

Requirements summary

Required education includes a registered nurse diploma or a bachelor's degree in a related field, along with a minimum of 3 years of experience involving direct patient contact.
