Intensive Care Coordinator

The coordinator will lead care planning teams, coordinating services for masshealth-Enrolled youth with serious emotional or behavioral health needs, and serve as a liaison between individuals, agencies, and supportive programs. Responsibilities also include identifying and helping families access services, monitoring effectiveness, providing trauma-Informed wraparound services, and assisting caregivers with advocacy skills.

Applicants must be at least 21 years old and possess a bachelor's degree in social work, psychology, or a related field, with a master's degree preferred. Candidates must maintain a valid driver's license and have access to a vehicle for transporting youth, while also being open to a flexible schedule including evenings.
