Case Manager - Caregiver Supports

This role involves providing specialized support to low-Income youth and young adults, often with backgrounds in foster care or homelessness, to help them develop independent living skills in areas like education, employment, and housing. The case manager must develop individualized, strengths-Based service plans, maintain thorough documentation, and build strong collaborative relationships within the community.

Candidates need a bachelor's degree in a related social services field or equivalent experience, with a master's degree preferred, alongside experience working with teens or young adults in relevant fields such as employment, foster care, or housing. Essential requirements include strong youth engagement skills, the ability to teach independence skills, basic computer proficiency, and the capacity to safely transport clients as an authorized driver.
